The Medical Card, explained

A Medical Card is one of the most valuable things you can hold in Ireland — it gives free GP visits, heavily subsidised prescriptions, free public hospital care and more. The catch is that it's means-tested, and a lot of people who'd actually qualify never apply because they assume their income is "too high". The income test is more generous than most people think — especially because the HSE deducts your rent, childcare and other costs first.

What it covers

A full Medical Card gives you:

Do you qualify?

It depends on your net weekly income compared to the HSE's limits for your household. The crucial part people miss: before the test, the HSE deducts a long list of costs from your income, including:

So your "income" for the test can be a lot lower than your gross wage. As a rough guide to the weekly income limits (net, after those deductions — confirm current figures on hse.ie):

If you're 70 or over, the test is separate and more generous, and many older people qualify automatically.

Even over the limit? You may still get a card on hardship grounds if you have high ongoing medical costs (a "discretionary" Medical Card), or qualify for the GP Visit Card, which covers free GP visits at higher income limits. Children under 8 and everyone over 70 can get a GP Visit Card more easily.

How to apply — step by step

1

Gather your figures

Recent payslips or income details, plus proof of your rent/mortgage, childcare and travel-to-work costs — these reduce your assessable income.

2

Apply online

The quickest route is the HSE's online application at hse.ie (or a paper form if you prefer).

3

Include your expenses

Don't skip the allowable expenses section — this is what brings a lot of working households under the limit.

4

Ask about the GP Visit Card

If you're just over the Medical Card limit, apply for the GP Visit Card instead — free GP visits with higher income thresholds.

5

Mention high medical costs

If you have significant ongoing medical expenses, flag this — a discretionary card may be granted even above the standard limit.

Common questions

Who is entitled to a medical card in Ireland?
Anyone ordinarily resident in Ireland whose income is within the HSE limits — assessed on net income after deductions like rent, childcare and travel to work. Over-70s have higher, simpler limits, and cards can also be granted on discretionary (hardship) grounds. Many working families qualify and never realise.
What are the income limits for a Medical Card?
They depend on your household and are based on net income after the HSE deducts costs like rent, childcare and travel to work. Over-70s have separate, higher gross-income limits. Because the deductions are generous, check the current figures on hse.ie rather than assuming you earn too much.
What's the difference between a Medical Card and a GP Visit Card?
A Medical Card covers GP visits, subsidised medicines, public hospital care and more. A GP Visit Card covers free GP visits only, but has higher income limits — so if you're just over the Medical Card threshold, you may still get the GP Visit Card.
I work full-time — am I automatically out?
Not necessarily. The test is on net income after major deductions (rent/mortgage, childcare, travel to work), so plenty of working households qualify. It's worth applying rather than assuming.
I'm over 70 — do I qualify?
There's a separate, more generous income test for over-70s, and many qualify. Even if you're above the Medical Card limit, over-70s can usually get a GP Visit Card.

This guide is general information, not medical or financial advice. Income limits and rules change — always confirm the current details and run the means test on hse.ie before relying on it.
