The scheme is run by the HSE. It is separate from the Treatment Abroad Scheme, which is only for treatment not available in Ireland, and from the Cross-Border Healthcare Directive, which pays back what the treatment would cost here.

Who can get it — the full rules
You must live in Ireland, be on a public or private waiting list for endometriosis surgery your consultant gynaecologist recommends, have no surgery date in Ireland, and complete the application with that consultant. A legal guardian can apply for a dependant aged 16 or over.
What it pays for
"The scheme covers your healthcare costs and basic air and sea fare costs (see below)." That means one pre-op appointment, one surgery with the hospital care during your stay, and the scans or tests the team abroad asks for. Depending on the centre, the HSE pays directly or you pay and claim it back.
You pay the economy fares upfront and are repaid when you come home. "You may also get basic air and sea fares for one travelling companion over 18 if your consultant gynaecologist recommends it." "Extra charges such as luggage, accommodation and food are not covered by the scheme and will not be paid."
How to apply
Download the ESAIS form from the HSE website, or call HSE Live on 1800 700 700 for a copy by post. You fill in part A and your consultant gynaecologist fills in part B, choosing a centre from the HSE's approved list and writing a referral letter. Send both to the HSE.
"The HSE must approve your application before you book any appointment, surgery or travel for the scheme." "The HSE will review your application and make a decision within 15 working days." A second surgery needs a new application.
