When it's paid
The death must have resulted from "An accident at work, An accident while travelling directly to or from work An occupational disease." "The person must have paid a minimum of one week's PRSI."

How to claim
Use the Death Benefit (Occupational Injuries) form and send it to Social Welfare Services in Longford, 0818 927 770, with "The original Death Certificate /interim Death Certificate A copy of the death notice A note of the funeral expenses."
